ABSTRACT 
Background/Aims: The aim of this study was to compare the clinical follow-up of hip arthroplasty (HA) performed in three different groups of patients: a group of chronic kidney disease (CKD) patients who were receiving long-term haemodialysis (HD group), a group of patients who had undergone renal transplantation (RT group) and a control group (CG) of non-renal patients.
Methods: A retrospective non-randomized controlled study was conducted of all HD and RT patients who, according to our Orthopaedic and Trauma Surgery database, underwent HA between January 1990 and December 2011. Each of these patients was compared with two non-renal controls (2:1).
Results: 114 HAs were reviewed: 20 in 18 patients on HD, 18 in 15 renal transplant recipients and 76 in 75 non-CKD patients. Mean age was significantly lower in the RT group (53.8±12.2 years) compared with both the HD group (75.0±12.5) and controls (68.0±16.3) (p<0.001). Cumulative early postoperative complications were more common in both the HD (40%) and RT groups (33%) compared with the CG (11.8%; p=0.024 and p=0.034, respectively). No differences were observed in the prevalence of late aseptic loosening. HD group presented a higher rate of periprosthetic fracture (10% vs 0%, p<0.05). The long-term cumulative failure rate of HA secondary to chronic infection was higher in the RT group (p<0.05).
Conclusion: In conclusion, both long-term haemodialysis and renal transplantation patients presented a higher rate of early postoperative complications regards to non-kidney-disease patients. Long-term hemodialysis patients also showed a higher rate of revision due to periprosthetic fracture and renal transplantation patients presented a higher rate of revision due to prosthetic joint infection.

Antecedentes / Objetivos: El objetivo de este estudio fue comparar el seguimiento clínico de la cadera artroplastia (HA) realizado en tres diferentes grupos de pacientes: un grupo de enfermedad renal crónica (ERC) de los pacientes que estaban recibiendo hemodiálisis a largo plazo (HD grupo), un grupo de pacientes que habían sido sometidos a trasplante renal (grupo RT) y un grupo control (CG) de los pacientes no renales.
Métodos: Estudio retrospectivo no aleatorio controlado se llevó a cabo de todos los pacientes HD y RT que, de acuerdo a nuestra base de datos de Cirugía Ortopédica y Traumatología, se sometieron HA entre enero de 1990 y diciembre de 2011. Cada uno de estos pacientes se comparó con dos controles no renales ( 2: 1).
Resultados: 114 ha fueron revisados: 20 en 18 pacientes en HD, 18 en 15 receptores de trasplante renal y 76 en 75 pacientes sin IRC. La edad media fue significativamente menor en el grupo RT (53,8 ± 12,2 años) en comparación tanto con el grupo de HD (75,0 ± 12,5) y controles (68,0 ± 16,3) (p <0,001). Complicaciones postoperatorias tempranas acumuladas fueron más comunes tanto en el HD (40%) y los grupos de RT (33%) en comparación con el GC (11,8%; p = 0,024 yp = 0,034, respectivamente). No se observaron diferencias en la prevalencia de la tarde aflojamiento aséptico. Grupo HD presenta una mayor tasa de fractura periprotésica (10% vs 0%, p <0,05). A largo plazo, la tasa de fracaso acumulado de HA secundaria a la infección crónica fue mayor en el grupo de RT (p <0,05).
Conclusión: En conclusión, tanto la hemodiálisis a largo plazo y los pacientes con trasplante renal presentan una mayor tasa de complicaciones postoperatorias tempranas respecto a los pacientes sin enfermedad renal. Pacientes en hemodiálisis a largo plazo también mostraron una mayor tasa de revisión debido a la fractura periprotésica y los pacientes con trasplante renal se presenta una mayor tasa de revisión debido a la infección articulación protésica.

Abstract

BACKGROUND:

The treatment of symptomatic acromioclavicular joint (ACJ) injury in the rotator cuff (RC) tear has not been well clarified.

PURPOSE:

To compare the clinical results between patients who had distal clavicle resection (DCR) and those who did not during RC repair.

STUDY DESIGN:

Randomized controlled trial; Level of evidence, 1.

METHODS:

From August 2008 to December 2009, a total of 56 consecutive patients (58 shoulders) were included. All patients had either a full-thickness or high-grade (>50%) RC tear, ACJ tenderness, arthritic change visible on plain radiographs, and a positive ACJ lidocaine injection test the day before surgery. Patients were randomized into 2 groups: DCR and RC repair (DCR+RCR group) and RC repair only (isolated RCR). Evaluation was performed preoperatively, at 6 months postoperatively, and at a final follow-up a minimum of 24 months postoperatively using the American Shoulder and Elbow Surgeons (ASES) score, the Constant shoulder score, range of motion examination, and pain visual analog scale (VAS).

RESULTS:

After simple randomization, 26 shoulders were allocated in the DCR+RCR group, and 32 were placed in the isolated RCR group. Five shoulders in the DCR+RCR group and 6 in the isolated RCR group were excluded from analysis due to loss of follow-up. Therefore, the evaluation was performed for 21 shoulders in the DCR+RCR group and 26 shoulders in the isolated RCR group. The mean follow-up period was 44.2 months in the DCR+RCR group and 44.0 months in the isolated RCR group. There were no differences in age, sex, symptom duration, RC tear size, or preoperative ASES, Constant, and VAS scores between the 2 groups (P > .05). At final follow-up, the ASES, Constant, and VAS scores were significantly improved in both groups (P < .001). There were no differences in ASES, Constant, and VAS scores between the 2 groups at final follow-up (P > .05), and there was no difference in residual ACJ tenderness (7 in the DCR+RCR group and 5 in the isolated RCR group) between the 2 groups (P = .270).

CONCLUSION:

There was no difference in the clinical evaluations between the combined arthroscopic DCR and RCR group and the isolated RCR group at a minimum 24-month follow-up. Arthroscopic DCR should be carefully considered in patients who have symptomatic ACJ arthritis with RC tears.

ANTECEDENTES:El tratamiento de la lesión sintomática articulación acromioclavicular (ACJ) en el desgarro del manguito rotador (RC) no ha sido bien aclarado. 
PROPÓSITO:Para comparar los resultados clínicos entre los pacientes que tuvieron una resección distal de la clavícula (DCR) y aquellos que no lo hicieron durante la reparación RC. 
DISEÑO DEL ESTUDIO:Ensayo controlado aleatorio; Grado de comprobación, 1. 
MÉTODOS:Desde agosto 2008 a diciembre 2009, se incluyeron un total de 56 pacientes consecutivos (58 hombros). Todos los pacientes tenían ya sea un espesor completo o de alto grado (> 50%) desgarre del RC, sensibilidad ACJ, cambio artrítico visibles en las radiografías simples, y una prueba de inyección de lidocaína ACJ positivo el día antes de la cirugía. Los pacientes fueron aleatorizados en 2 grupos: DCR y reparación RC (grupo DCR + RCR) y reparación de RC solamente (aislado RCR). La evaluación se llevó a cabo antes de la operación, a los 6 meses después de la operación, y en una final de seguimiento mínimo de 24 meses después de la operación utilizando el Hombro y Codo Cirujanos Americanos (ASES) puntuación, la puntuación hombro constante, la amplitud de exploración del movimiento y dolor escala analógica visual (VAS). 
RESULTADOS:Después de aleatorización simple, 26 hombros fueron asignados en el grupo DCR + RCR, y 32 se colocaron en el grupo de RCR aislado. Cinco hombros en el grupo DCR + RCR y 6 en el grupo de RCR aislado se excluyeron del análisis debido a la pérdida del seguimiento. Por lo tanto, la evaluación se realizó durante 21 hombros en el grupo DCR + RCR 26 y los hombros en el grupo de RCR aislado. El período medio de seguimiento fue de 44,2 meses en el grupo DCR + RCR y 44,0 meses en el grupo RCR aislado. No hubo diferencias en la edad, el sexo, la duración de los síntomas, RC tamaño lágrima o preoperatoria ASES, Constant y EVA entre los 2 grupos (p> 0,05). Al final del seguimiento, los ASES, y EVA constantes mejoraron significativamente en ambos grupos (p <0,001). No hubo diferencias en las evaluaciones clínicas entre el grupo combinado DCR y RCR artroscópica y el grupo RCR aislado en un mínimo de 24 meses de seguimiento. Artroscópica DCR debe ser considerado cuidadosamente en pacientes que tienen artritis ACJ sintomático con desgarres RC .
