Abstract

BACKGROUND:

Anatomic factors, including patella alta, increased tibial tubercle-trochlear groove (TT-TG) distance, rotational deformities, and trochlear dysplasia, are associated with dislocation of the patella. Identifying the presence of these anatomic factors both in isolation and in combination may influence treatment in patients with patellar dislocation.

PURPOSE:

The aim of this study was to compare the prevalence and combined prevalences of these anatomic factors using magnetic resonance imaging in a group of patients with and without histories of recurrent dislocation of the patella.

CONCLUSION:

Recurrent patellar dislocation is associated with an increased prevalence of patella alta, increased TT-TG distance, rotational deformity, and trochlear dysplasia compared with patients with no histories of patellar dislocation. Multiple anatomic factors were identified in the majority of patients with recurrent dislocation. Further research may identify which factors play a greater role in patellar stability and may allow physicians to predict which first-time dislocation patients are more likely to sustain recurrence.

Resumen

ANTECEDENTES:
Los factores anatómicos, incluyendo rótula alta, aumento de la distancia tibial surco troclear tubérculo (TT-TG), deformidades rotacionales, y displasia troclear, están asociados con la dislocación de la rótula. La identificación de la presencia de estos factores anatómicos, tanto en aislamiento y en combinación puede influir en el tratamiento en pacientes con dislocación de la rótula. 
PROPÓSITO:
El objetivo de este estudio fue comparar la prevalencia y las prevalencias combinadas de estos factores anatómicos utilizando imágenes de resonancia magnética en un grupo de pacientes con y sin antecedentes de luxación recidivante de la rótula.
CONCLUSIÓN:
Luxación rotuliana recurrente se asocia con un aumento de la prevalencia de la rótula alta, aumento de la TT-TG distancia, deformidad rotacional, y displasia troclear en comparación con los pacientes sin antecedentes de luxación patelar. Se identificaron factores anatómicos múltiples en la mayoría de los pacientes con luxación recidivante. La investigación adicional puede identificar qué factores juegan un papel más importante en la estabilidad de la rótula y pueden permitir a los médicos predecir qué dislocación por primera vez los pacientes son más propensos a sufrir recurrencia.

Abstract

BACKGROUND::

Existing radiographic classification schemes (eg, Tönnis criteria) for DDH quantify the severity of disease based on the position of the ossific nucleus relative to Hilgenreiner's and Perkin's lines. By definition, this method requires the presence of an ossification centre, which can be delayed in appearance and eccentric in location within the femoral head. A new radiographic classification system has been developed by the International Hip Dysplasia Institute (IHDI), which uses the mid-point of the proximal femoral metaphysis as a reference landmark, and can therefore be applied to children of all ages. The purpose of this study was to compare the reliability of this new method with that of Tönnis, as the first step in establishing its validity and clinical utility.

CONCLUSIONS::

The IHDI method of classification has excellent inter-rater reliability, both among experts and novices, and is more widely applicable than the Tönnis method as it can be applied even when the ossification centre is absent.

 Los esquemas de clasificación radiográficos
existentes (por ejemplo, criterios Tönnis) para DDH cuantificar la gravedad de la enfermedad basándose en la posición del núcleo de osificación en relación con Hilgenreiner de líneas y de Perkin. Por definición, este método requiere la presencia de un centro de osificación, que puede ser retrasada en apariencia y excéntrico en una ubicación dentro de la cabeza femoral. Un nuevo sistema de clasificación radiográfica ha sido desarrollado por el Hip Internacional Displasia Instituto (IDH-D), que utiliza el punto medio de la metáfisis femoral proximal como un hito de referencia, y por lo tanto se puede aplicar a los niños de todas las edades. El propósito de este estudio fue comparar la fiabilidad de este nuevo método con el de Tönnis, como el primer paso para establecer su validez y utilidad clínica. 
Conclusiones :
El método IDH-D de clasificación tiene una excelente fiabilidad entre evaluadores, tanto entre expertos y novatos, y es más ampliamente aplicable que el método Tönnis ya que puede aplicarse incluso cuando el centro de osificación está ausente.
